Password, Sign in and Account Help

I forgot my username or password. What should I do?

If you didn't receive your 2-step verification code, make sure you’ve checked your spam folder and all email accounts. If none of your accounts received the email or you have been locked out of your account,fill out this formand one of our team members will assist you.

I didn't receive my 2-step verification code. What should I do?

Try checking the spam or junk folders in your email. If the email with your code is not there, try clicking Resend Code.

If you still didn’t receive the email, it could be that your account has a different email address on file. If you have multiple email accounts, check one of the others to see if the verification code was sent there instead.

If none of your accounts received the email, it could be that we don’t have an email address on file. If this is the case,fill out this formand one of our team members will assist you.

I was logged out of My Baptist Chart. What happened?

We aim to protect your privacy and information. If you remain idle for 10 minutes or more after you log in to My Baptist Chart, you will be automatically logged out. We recommend that you log out of My Baptist Chart if you need to leave your computer for even a short period of time.

I got locked out of my account. What should I do?

To have your account re-activated,fill out this formand one of our team members will assist you.

I have MyChart accounts with other organizations. How do I link them?

If you've been seen at another health care organization, you might be able to view information from that medical record right in My Baptist Chart. You might have heard of this feature referred to as Happy Together. This includes information from other organizations, such as: allergies, care team, health issues, medications, messages, test results, visits

To view this information, you must link your account. Check out ourvideo tutorialon how to do this.

I'm not getting any notifications.

On the My Baptist Chart website or in the MyChart app, go to settings to adjust which notifications you see.

Can I personalize my notifications from MyChart?

Yes, you can by going to “communication preferences.”

How do I delete my account?

How do I sign up?

There are several different methods of My Baptist Chart signup:

  • Clinic staff might sign you updirectly while you're at the front desk or in the exam room.
  • You may receive a My Baptist Chart activation code on your After Visit Summary or on a billing statement.
  • You may receive a text message or email with an activation code when you come in for a visit.

No activation code?
No problem. You can still easily self-signup online to create a My Baptist Chart account. You must be 18 years old and request an activation code. Follow these easy steps to request a code:

  1. Scroll up to the Sign in section at the top of this page and click "Sign up now"
  2. Click "Sign up online"
  3. Click "Match Yourself with Our Records" or "Verify with a Third Party" to supply personal verification items, such as the last four digits of your Social Security number and your date of birth, and have an activation code sent to your email address or mobile phone.

Can I get a new activation code if I lost it, let it expire or didn't receive it?

If you don’t have an activation code, you can request one on thesign-up page.

My activation code does not work. What should I do?

For your security, your activation code expires after a set period of time and is no longer valid after the first time you use it. You can request an activation code on thesign-up page.

My Baptist Chart for Your Family

Can I view the My Baptist Chart records of people I am a caregiver for?

Yes, you can. My Baptist Chart proxy access allows you to access the medical records of your family members and others you care for, with their permission. You must be at least 18 years old to have proxy access to another person’s medical record.

What is Proxy Access?

Proxy access allows parents, legal guardians and caregivers to log into their personal MyChart account and connect to the health information of a patient they represent. As a proxy, you can view upcoming appointments, request appointments, view lab results, email a doctor on the patient’s behalf, view medications, request prescription refills and view health reminders.

Instructions

After completing the Portal Access Request Form, you can return it to your care team at your next appointment, or mail it to the address on the form.

Children's Records Access Varies with Age
Adults with legal guardianship rights to a child (younger than 18 years old) may request proxy access to the child's My Baptist Chart account. The level of access is determined by the age of the minor.

  • 0 - 11 years old:Parents have full access to their child’s record and My Baptist Chart account.
  • 12 - 17 years old:Parents have partial access to their child’s record and My Baptist Chart account. Adolescents in this age range may also access their own My Baptist Chart after filling out and signing a portal access request form.
  • 18+ years old:Patients have full access to their own accounts.

Each adult must have their own My Baptist Chart account for accessing the child's My Baptist Chart account and complete thePatient Portal Access Request Form-Child/Adult or the Patient Portal Access Request Form-Adolescent/Emancipated Minor indicating minor patient access. Once the child reaches age 18, the proxy access authorization will automatically expire and the adult will no longer have access to the child's My Baptist Chart account. At that time, to still access your adult child's account, a new Patient Portal Access Request Form must be completed and signed indicating Adult Patient access.

Granting Another Person to Access Your Account

In reverse, you might also want to grant a family member or friend access to your medical records when you need assistance managing your appointments and other medical needs. To give someone access to your medical record, grant access via a proxy invite through the Friends and Family Access page in My Baptist Chart.

Does my proxy access from the old portal transfer to My Baptist Chart?

If you had proxy access to another person's account in the old portal (My Baptist Connect), you will need to re-authenticate yourself usingthis Portal Access Request form, activate their new My Baptist Chart account, and then connect it to your My Baptist Chart account.

Can my spouse and I share one My Baptist Chart account?

No, due to the sensitive nature of medical information, each person must have their own My Baptist Chart account.

Why can't I see my bill for my child?

To see your child's bills, you must be listed as the guarantor. Contact your provider to gain access.

Your Medical Record

When can I see my test results in My Baptist Chart?

With My Baptist Chart, you can access your visit details, lab results, and medical history in one place, as soon as they become available, which may be before your doctor has a chance to review them. It’s always your choice to view these results now or wait to discuss with your care team.

After your provider reviews your results, you might see additional comments and interpretation inMy Baptist Chart.

Important things to consider when viewing test results in My Baptist Chart.

When will I see my medications, immunizations and health conditions?

Your provider will review your medical record prior to any upcoming appointments and will populate medications, immunizations and any known health conditions to your My Baptist Chart account. You may not see that detail in your account until you have an appointment scheduled with a Baptist Health provider.

If I send a message to my provider, when can I expect a reply?

You will generally receive a response within 1-3 business days. Note that My Baptist Chart messages should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

What if I have a question about my scheduled appointment?

Please call your provider's office.

Where can I update my personal information?

On the Personal Information page in My Baptist Chart, you can update your address, phone number, email address, preferred name, and other personal details at any time so that your clinic always has the most up-to-date information in your record.

What should I do if some of my information in My Baptist Chart is incorrect?

Your My Baptist Chart information comes directly from your electronic medical record at your provider's office. Ask your provider to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record after each visit.

My Baptist Chart offers personalized and secure online access to your medical records. It enables you to manage and receive information about your health. With My Baptist Chart, you can:

  • Schedule medical appointments.
  • View your health information, including medications, allergies, test results, and more.
  • Request medication refills.
  • Access resources for trusted health information.
  • Message your care team.

Is there a fee to use My Baptist Chart?

No, My Baptist Chart is a free service for all patients.

What technology do I need?

You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Edge, Chrome, Firefox, or Safari).

You can also access My Baptist Chart in ourBaptist Access Appor the MyChart app App Store |Google Play Store.

How is My Baptist Chart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account must be accessed using that password. On mobile, the account may also be accessed using face ID or fingerprint login. Unlike conventional email, all My Baptist Chart messaging is done while you are securely logged in to our website or mobile app.

You can ensure that your account stays secure even if someone else has your username or password by turning on two-step verification. When this feature is turned on, you must enter a code that is sent to you by email or text message to log in to My Baptist Chart, in addition to using your username and password.

Transition from previous portal to My Baptist Chart

Will my data and records from the previous portal move to My Baptist Chart?

Yes, your Baptist Health records and data will still be available in this new portal.

However, any self-published data will not be transferred. For instance, if you added any over-the-counter medication or allergy information to My Baptist Connect for your own record-keeping, this information will not transfer to My Baptist Chart.
